Federal School of Surveying, Oyo, credential/registration forms submission notice to concerned ND, HND, PD, PPD, and PGD GIS students. Scroll down to read in details.
This is to inform all students especially the 2019/2020 and 2020/2021 ND, HND, PD,PPD, and PGD GIS students to submit the hard copies of all the Registration Forms to the Admissions and Records Office on or Before 3.00p.m, on 16th Monday, August 2021 unfailingly.
Please find below the necessary documents and forms to be submitted for all categories of Students.
- Receipt for payment of Acceptance Fee.
- Photocopy of Acceptance Letter from the School Portal.
- Copies of Forms A-Personal Details.
- Course Registration Form (Form B-Blue Form).
- Form C (Medical and Accommodation Records).
In addition to the above, the following documents are to be submitted as highlighted below.
- ND students should bring both originals and photocopies of:
- O/Level result(s)
- JAMB admission letter
- JAMB result slip
- Birth certificate
- Local Government Identification letter
- Letter of good behaviour from parents, guardian or sponsor
- 6 Passport photographs
- HND students should bring both originals and photocopies of
- O/Level result(s)
- JAMB admission letter
- JAMB result slip
- Birth certificate
- Local Government Identification letter
- Letter of good behaviour from parents, guardian or sponsor
- 6 Passport photographs
- Postgraduate (PPD, PD and PGD) students should bring both originals and photocopies of:
- O/Level result(s)
- JAMB admission letter
- JAMB result slip
- Birth certificate
- Local Government Identification letter
- Letter of good behaviour from parents, guardian or sponsor
- Passport photographs
The ND II, HND II, PD and PGD and all outgoing students should note that submission of the above documents and signing of the Matriculation Register is a pre-requisite for collection of Notification of Results
Every student should also submit the photocopy of the TISHIP and for record purposes at the Admissions and Records Office.
