Eligibility

Applicants must meet all the given criteria:

  • A United States citizen, or a legal and permanent resident of the United States
  • Diagnosed with inflammatory diseases, including Crohn’s disease or ulcerative colitis, by a healthcare professional
  • Enrolling in or currently attending an accredited US institution (e.g., college, university, or trade/vocational school) for education during the upcoming 2020-2021 academic year.
  • Applicants must have completed high school or obtained a General Education Development (GED) certificate before the end of June 2020.
  • Students are eligible who diagnosed with Crohn’s disease (CD), ulcerative colitis (UC), rheumatoid arthritis (RA), juvenile idiopathic arthritis (JIA), psoriatic arthritis (PsA), ankylosing spondylitis (AS), uveitis (UV), hidradenitis suppurativa (HS) or psoriasis (Ps).
  • Eligibility restricted to patients diagnosed with Crohn’s disease or ulcerative colitis.

Supporting Documents:

  • One online letter of recommendation completed on their behalf. Applicants are responsible for making sure their recommender submits a letter of recommendation on or by the letter of recommendation.
  • All applicants will be required to write an essay of no more than 2,000 characters. An essay answering one of the following topics:
  1. Based on your experience with your inflammatory disease, what inspirational advice would you give to others living with that same disease entering high school or college?
  2. Life can be full of challenges or opportunities, depending on your outlook. How has your diagnosis changed your outlook and helped turn challenges or complicated tasks into opportunities?
  3. What have you learned about yourself during your journey living with your disease? How will you use that learning to tackle the new adventure ahead as you pursue a degree?

Benefits

The program will provide up to $15,000 to each winning candidates that dependent upon the type of degree pursued.

  • Associate degree: $5,000
  • Bachelor degree: $15,000
  • Masters/PhD: $15,000

Application Deadline: Thursday, December 5, 2019, at 3:00 p.m. CT. All applicants will be notified of their status by January 20, 2020.
